The Revised PhilHealth Member Registration Form (PMRF)

To All Employers and Members;

Please be informed that the PhilHealth Circular No. 2020-0001 on "The Revised PhilHealth Member Registration Form (PMRF)", is now posted in our website. 

The revised PMRF is to simplify registration and updating procedures. It shall be used to facilitate registration of all Filipinos and ensure an accurate database consistent with the Universal Health Care (UHC) provisions on new Membership Types such as Direct and Indirect Contributors.

This Circular shall take effect fifteen (15) days after publication in any newspaper of general circulation.
